To me, this appears to be a rather poor "photo-chop" and not at all real. Think about it, there is not even a smear of blood anywhere, if this were real, there might not be blood gushing everywhere, but there WOULD be some blood, I would estimate at least 300-500 cc's, the penis would not remain erect and straight like that either.
The guy may really be penectomized. He looks a lot like some pictures someone linked to here a few months ago, (from the Castrated She-Male group maybe.) Of course one cockless guy from the waist down can look a lot like another cockless guy from the waist down, so I wouldn't swear to it.
But the picture with the severed penis is totally bogus. DeaconBlues was right on the money. In grad school I worked evenings in surgery and hauled a lot of severed body parts down to the path lab. No way could that napkin be spotless no matter how tightly the penis is banded. And look at the skin around where the penis would have been cut. Even high precision plastic surgery leaves the operating area looking like hamburger. Theres not even a sign of swelling or blood.and he is holding what looks like a cotton ball. If he had just had it lopped off it wouldn't look like that.

It's really not that difficult to figure out. Unless he meticulously cleaned the knife, drained that detached penis of all it's blood, etc. there is no way this could be a real photo of a penectomy. Not to mention that dick has been cauterized and there is no cautery device present in the photo yet all the other implements are prominently displayed.
I vote for faked.

As a photoshop expert, I assure you that this picture is indeed doctored. There are several tell tale signs, but the most obvious one is that 'severed' penis still looks healthy as if blood is still flowing through it (the veins would not be blue unless they were still returning low-oxygenated blood back through the circulatory system).
There are several other tell tale, but I'll keep those photoshop secrets to myself.
Just looking at it screams FAKE, were is the blood (way to clean), dick is not blue from lack of oxygen and is still hard (no band showing).
