    The Pros and Cons of Hormone Replacement Therapy

    The American Medical Association Encyclopedia of Medicine defines Hormone Replacement Therapy as: “The use of a synthetic or natural hormone to treat a hormone deficiency…”

    The hormone Testosterone is greatly reduced in the blood of a Eunuch after castration. The remaining sex hormones in the body are produced by the adrenal glands and are only about 5% of the pre-castration level in an adult. Some Eunuchs choose Hormone Replacement Therapy after their surgery. Either the male hormone testosterone or the female hormone estrogen can be used.

    Testosterone – Pros: Testosterone has the benefit of increasing the bone and muscle mass in an adult, as well as providing a strong libido. The male hormone also provides greater stamina and maintains male secondary sexual characteristics.

    Testosterone – Cons: Testosterone has been linked to the advancement of bone and prostate cancer. It causes male pattern baldness and is known to increase male aggressiveness. It has been suggested that testosterone is the reason males live on average 10 to 15 years shorter lives than Women and Eunuchs.

    Estrogen – Pros: Estrogen has the benefit of maintaining some libido, although diminished for an adult male. It also maintains bone mass and may prevent osteoporosis. Lower levels cause no or very little feminization of the body. Higher levels can cause moderate increase in breast size and body shape.

    Estrogen – Cons: This is a female hormone being taken by a relatively male body. The changes in attitude and feeling for the non-transgendered may not be acceptable. Estrogen, however, is a far milder hormone than testosterone and causes fewer negative health effects. All HRT must be monitored (testosterone or estrogen) because they may both cause some liver damage. HRT should always be monitored by a doctor.

    General HRT Pros and Cons: In general there are three camps on HRT. Transsexuals need HRT in order to modify their bodies to become female. Eunuchs are split into two. The Eunuch who wishes to maintain some or all of his sexuality would most likely use Testosterone. He would be as normal as any adult male in appearance and feeling, and subject to all male ills associated with this hormone. He may also choose to use estrogen. Again, there are few negatives to estrogen. This would stop most health problems associated with no HRT.

    Finally – No HRT

    Pros: If you choose not to take HRT, you will develop to varying degrees (depending on age of castration) certain features. Your skin will become softer, the hair as well. You will develop the “eunuch calm”, an indescribable feeling of peace and calm.

    Cons: You may gain weight (10 to 50 pounds depending upon your metabolism). You will also be at a greater risk for osteoporosis and should take regular calcium supplements for life (1,500 mg per day).

    Thank you for posting this. Very helpful.

    Now, would I just go see a physician for the HRT? Would they put you through a large battery of questions? I want to go through proper channels but don't want them to force me into visiting a psychiatrist long term or something of the sort.

    FYI folks, let me begin by saying I wish you all the best in your quest. I was castrated in 2007 and have been very happy with the results, and very happy in general since. I hope Dr. Arnkoff proves to be a reliable resource to the community.

    One thing I notice is that a lot of folks have asked about how to get testosterone, what about complications, etc. Please understand that like that tattoo, this is not a reversible process! You will need to be prepared to talk to your own doctor about it afterwards for sure, and I recommend talking to him or her BEFORE if possible. I talked with my primary care doc about hypersexuality issues, and he was very supportive about the problem. However, he did not have any suggestions for resources. At my first visit post-castration, he was downright apologetic for not addressing the issue more aggressively. And since then, he has been quite helpful.

    So remember, you're going to have to tell your doc about it sooner or later, and sooner is better!

    That is good information. I have no aversion to speaking with a primary care doctor whatsoever. I simply don't want to get in the situation where I can't obtain a source of testosterone post procedure. I am definitely not rushing into this decision or taking it lightly. However, so far I am definitely leaning heavily towards getting it completed. I believe I will speak with Dr. Arnkoff this week. I don't want to get into the situation either where I wait too long and he stops performing the procedures.

    I have my 2k saved up and am going to call and make an appointment, however Can anyone tell me if there are hotels that have airport shuttles available? Not familiar with the area as I'm from Nebraska any help would be appreciated.

    Has anyone made an appointment with Dr. A for a Saturday morning in March? I'm scheduling an appointment and going alone. Thought I might see who else will be there when I pick a weekend.

    Nope, it's supposed to be a 30 to 40 minute drive from the airport (Detroit) and none of the local hotels (those in Southfield) that I've found have an airport shuttle. Cab rides look to be about $60.00 to $70.00 each way and then you're stranded. Soo.... there are some rental car companies offering a decent two day rate if you book 'em early enough, don't know if they have to be pre-paid as well. Most of the hotels offer a better rate if booked at least two weeks in advance and paid for at that time but they are non-refundable as well.

    I used the marriot Courtyard which is about 5 to 10 from his office.. Car rentals are the way go. The Dr. will give you two perscriptions...one for pain pills and one for antibiotics. There is a Walgreens about five minutes from his office. I stayed overnight and used an icebag to reduce the swelling and flew home to Calif. the next day. Only took the pain pills for 3 days. However, I took all of the antibiotics as perscriped. Totally satisfied wht the doctor and the procedure. Sorry I didn't have it done sooner.

    Not on HRT>.

    Similar to my experience regreen13. I took the full course of the anti-bacterial (cephalexin) to guard against any urological infection but because I experienced no pain post-op, the painkillers (hydrocodone) were not taken (they can also cause constipation). Be aware that the Walgreens store next door to the clinic may be open but the pharmacy inside has different opening times (I had to return 2 hours later to pick up the prescription). Make sure you have purchased a jock strap/athletic support before surgery (not on sale in Walgreens) and purchase plenty of sterile gauze pads and anti-biotic pain relief cream (Neosporin) for the incision healing.
    Get a room close to an ice making machine if possible so you can replenish the icepacks applied to the scrotum regularly.

    I talked to Dr Arnkoff twice last Fall about having an orchiectomy last November, but I had to postpone it because I wasn't able to bundle a second week together needed for an adequate recovery. I don't want to end up in the ER and trying to explain want happen to my nuts. My next window for having orchiectomy is at the end of June.

    I don't want to push this off because I don't want to take the chance that something happens to Dr Arnkoff and have to spend twice the money with Dr. Farah. My experience with Dr Arnkoff was very positive and if I'm going do this, I rather have it done by a doctor, so Dr Arnkoff is the way to go.

    I was castrated by Dr Arnkoff on February 4th and am very happy with the results. (I guess I am going to have to change my user name :-) )

    I highly recommend Dr Arnkoff vs going to a cutter or non physician. The entire procedure took about an hour. Three weeks later the ends of the cords are still swollen a bit, but have gone down quite a bit in the last week. The last of the dissolvable stitches came out last night.

    In about six months, I am going to go back to have the scrotum removed.

    My husband and I are very happy with the results and highly recommend Dr Arnkoff.

    I too recommend Dr Arnkoff. I called and talked to Dr. Arnkoff twice last Fall and found him to be both positive and sensitive to people like us. I plan on using Dr. Arnkoff when the time comes for me to become a real eunuch rather then be just another guy with low T-Levels, who want his balls gone for good.

    That one question that I forgot to ask Dr Ar. Arnkoff was how much does he charges to remove my scrotum.

    Does anyone knows what he charges to remove the scrotum?
