For most males the most sensitive area is around the corona of the glans, particularly the underside. You probably know where it feel the best to masturbate. An almond sized patch of this area is excised keeping the nerves intact (We use the term nerves to apply both to the sensors located in or just under the skin, and the nerve fibers that run back to the spine and on to the brain) This patch becomes the clitoris. The vagina is usually constructed of tissue from the scrotum (which is why a few surgeons will not do SRS where the scrotum is removed). If scrotal tissue is insufficient split thickness grafts from elsewhere on the body are used to make up the difference. The skin of the penis itself is used to form the labia. In another technique the vaginal canal is fabricated from a colon graft, but the labia and clitoris are fabricated the same.
